Clinical Electrophysiology: from ECG to CAGE

The objective of the two days is the updated review of the main conduction disorders and supraventricular tachycardias from diagnosis to treatment in dogs and cats.

The initial part of the conference will be focused on self-assessment on intra-atrial and intra-ventricular conduction disorders. This section is a practical workshop performed with Slido. During the section, various electrocardiographic tracings will be proposed to be analyzed with the help of multiple-choice questions. The next session is aimed at the theoretical illustration of intra-atrial conduction disturbances such as conduction delays of the Backman’s fascicle and conduction blocks of the Backman’s fascicle. Subsequently, the vectorial alterations present in the case of conduction disorders such as monofascicular blocks (right bundle branch block, anterior fascicular block), bifascicular (complete left bundle branch block, right bundle branch block associated with anterior fascicular block), trifascicular will be explained.
Based on recent publication, the different electrocardiographic patterns of ventricular pre-excitation in the dog will be illustrated. Ventricular pre-excitation is a syndrome characterized by short PQ interval, presence of delta wave, wide QRS complex, repolarization phase alterations. The analysis of the delta wave and the QRS complex allows the localization of the accessory pathway around the tricuspid annulus, distinguishing anterior accessory pathways from posterior accessory pathways, facilitating interpretation and ablative therapy during endocardial mapping. After an initial theoretical explanation, the day continues with a proposal for diagnostic algorithm test by all participants for the purpose of validation followed by publication of the algorithm itself.

In the next session, different supraventricular tachycardia will be illustrated. Supraventricular tachycardias are classically characterized by narrow QRS complex, with different signs of atrial deflection based on the tachycardia underlying mechanism. After an initial theoretical section, a proposal for diagnostic algorithm test by all participants will be proposed for publication. On the second day, topics of great interest and recently published will be addressed, such as the influence of the autonomic nervous system on the firing from the pulmonary veins and on the atrial myocardium. This topic will be addressed from the etiopathogenetic point of view and from the diagnostic point of view by means of the analysis of electrocardiographic tracings and by means of dynamic analysis with the interpretation of Holter monitoring.
After these explanations, new therapy topics in the electrophysiological field will be addressed, such as the 3D electrical mapping of the atrial and ventricular myocardium in order to precisely identify the origin of rhythm disturbances and treat them through the use of ablative techniques. In the last two section a review on supraventricular tachycardias with a practical workshop will be held to review all the electrocardiographic findings and their relationship with the underlying electrophysiological mechanism.

DVM, Dipl. ACVIM (Cardiology) Relatore
Dr. Hsue completed veterinary school at the University of California – Davis, a small animal rotating internship at The Ohio State University, and a cardiology residency back at the University of California – Davis, after which he became board-certified in cardiology by the American College of Veterinary Internal Medicine. He then underwent a translational cardiac electrophysiology research fellowship at the University of Pennsylvania, where he studied animal models of arrhythmic diseases. He is currently an Assistant Professor at Cornell University’s College of Veterinary Internal Medicine. His current research interests include cardiac electrophysiology, electroanatomic mapping, three-dimensional echocardiography, and cardiac focused ultrasounds.

Roberto Santilli graduated from the College of Veterinary Medicine of the University of Milan in 1990. He became a diplomate of the European College of Veterinary Internal Medicine - Companion Animals (Specialty of Cardiology) in 1999. Between 2004 and 2006, he completed a Master in Electrophysiology and electrical stimulation at the University of Medicine of Insubria. He then obtained a PhD at the University of Turin – College of Veterinary Internal medicine in 2010 completing a research on diagnosis and treatment of supraventricular tachycardias in the dog. Roberto Santilli is the head of the cardiology departments of the Clinica Veterinaria Malpensa, Anicura in Samarate, Varese (Italy). Since 2014, he has been an Adjunct Professor of Cardiology at the Cornell University College of Veterinary Medicine, where he is actively involved in the activities of the cardiac electrophysiologic laboratory. He’s author of 68 publications on peer-reviewed journals on arrhythmias’ topic and co-author of the book: Electrocardiography of the dog and cat”, now translated into 6 languages. His main research activities include the diagnosis and treatment of arrhythmias in dogs and cats.

Manuela Perego si è laureata presso l’Università di Milano nel 2003. Nel 2003 ha iniziato una collaborazione presso la Clinica Veterinaria Malpensa - Anicura (Samarate-VA) e nel 2012 presso l’Ospedale I Portoni Rossi - Anicura (Zola Predosa-BO) occupandosi di ecocardiografia, aritmologia e di procedure di cardiologia interventistica. Nell’anno 2019 ha ottenuto il Diploma Europeo di Medicina Interna Specialità di Cardiologia. È autrice di articoli inerenti la cardiologia e l’aritmologia su riviste italiane e internazionali, del libro Elettrocardiografia del cane e del gatto (2009, Elsevier-2018 Edra) e co-autrice del libro Manuale di Cardiologia del cane e del gatto (2012, Elsevier). È stata relatrice a diversi Congressi Italiani e Internazionali.
